2020 Provider of the Year Award Nomination

The Harrington HealthCare Provider of the Year recipients will be selected by a committee made up of the Chief Medical Officer, the Medical Staff President, and the previous years’ winners.

Nominations will be judged on:

• Reasons why a Provider was nominated
• Number of nominations received
• Active medical staff membership (required)

Providers, employees, volunteers, patients, and board members of Harrington HealthCare System are eligible to submit nominations (one nomination per person).

The entire form must be filled out in order to be eligible for submission.

The recipient will be announced in a ceremony at the Employee Gala in May.
