I have granted permission to use my image or likeness, in whole or in part, for any and all visual and audio material throughout the United States and the world, including but not limited to, print and broadcast media, digital media and outdoor advertising. I also give permission for Agency, or a vendor working at their direction, to alter and composite my likeness without restriction and without my inspection or approval. I waive the right to inspect or approve such materials created and/or used by or on behalf of Agency.

  I release Agency, their respective assigns and successors, from any and all claims that may arise from the use of my image or likeness, including but not limited to, claims related to defamation, invasion of privacy, false lights, infringement of moral rights, rights of publicity or copyright. I hereby attest that I have read, and agree to the above statement. This agreement expresses the complete understanding of the parties.
